We are pleased to open registration for Vacation Bible School 2009! Plan to join us at Camp Edge/VBS 2009 where we will experience and discover God everywhere!

WE STRONGLY ENCOURAGE YOU TO ENROLL EARLY! We eagerly anticipate the opportunity to spend the week with your children! Please note the following:
- We are enrolling children, ages 4 to 12 for Vacation Bible School
- The registration deadline is Thursday, June 18. Your prompt registration enables us to properly staff and equip our program and ensure that every child receives quality VBS materials in a timely manner.
- Please mail or bring completed registration forms (one per child) and fees to the Church Office. Make checks payable to “Bethany Christian Church” with the notation of “VBS” in the memo line.
- Registrations received after June 18 will be placed on a waiting list. Please register early to ensure your child’s participation.
- VBS is for grown ups too! Join in the fun and volunteer to help out during VBS! Please complete the VOLUNTEER FORM indicating your availability and area(s) of interest, and return it to the church office. A nursery will be provided for children (age 3 and under) of VBS volunteers.

Enrollment Forms
To register your child, please turn in the following items to Vivian Holz in Bethany Christian School or to Doug Threeton in the Church Office.
a. Completed registration form (PDF) – one per child
b. Emergency Medical Authorization sheet (PDF) – one per child
c. Consent to participate sheet (PDF) – one per child
d. Registration Fee: Early Registration fee of $35.00 per child (before June 18th)
OR
Regular Registration fee of $60.00 per child (for placement on waiting list – after June 18. **Please note that if space does not become available for a wait listed child, your fees will be returned to you).
Forms are also available in the Church office.
In addition to the registration form, there are two other required forms which were adopted by the Official Board of the Church. The following are a few points to aid you in completing these forms:
1. Emergency Medical Authorization – As you know, if your child becomes ill, or is injured while participating in a church activity, and if you cannot be located, neither a hospital nor a doctor would treat your child, for to do so without parental consent could subject the doctor or hospital to a lawsuit. This form allows the Bethany Vacation Bible School Staff to act in your behalf until you are located. Remember this is for your child’s benefit.
a. Use complete names throughout the form (first, middle, and last).
b. Either type or print in ink.
c. The form must be signed by one or both parents.
d. If you desire, you may add the name of your health and accident insurer on the back.
2. Consent to Participate – This simply tells the Church that you know your child is attending Bethany Vacation Bible School and that you approve.

We need volunteers to help make VBS a success. If you would like to volunteer, please fill out the volunteer form (PDF) and return it to the church office.
